Position Summary

To promote quality, cost-effective health care outcomes for the patient /family. To work in a collaborative manner with the clinical team, physician, and insurers for utilization management, care coordination, discharge planning, and evaluate options and services to meet an individual’s health needs.

Minimum Education Requirements

Graduate of accredited school of nursing

Minimum Experience Requirements

Case management and 1 year homecare with Oasis experience required. 1 year experience with regulatory coding experience preferred.


Kansas License Registered Nurse (RN)

Cardiopulmonary Resuscitation (CPR) required, Basic Life Support (BLS) from American Heart Association required for

facilities with an Automated External Defibrillator (AED) preferred for facilities without an AED.

Current CO Drivers license and auto insurance (Field staff )


Position Duties (Essential Functions)

Assessment, Intervention, and Critical Thinking Skills

Interviewing, monitoring, negotiation, data gathering, and expert

clinical intervention skills. Broad array of skills to include:

simple problem-solving to scientific inquiry and reflective

judgment related to patient diagnosis, prognosis, care needs,

realistic goals. Knowledgeable resource advisor related to

discharge plans and goal achievement.


Evaluation / Analysis Skills

Outcome monitoring and data collection with resultant tracking

and trending analysis for strategic planning. Cost containment

procedures and strategies. Program evaluations and research

(e.g., recidivism, satisfaction). Knowledgeable about utilization

management models. Recommends CQI process in areas

identified through CM as not meeting desired outcomes.


Interpersonal Relationship and

Teaching Skills

Values and behaviors that demonstrate care for patients and

other stake holders internal and external to the organizations.

Sensitivity to individual cultures and preference. Patient focused

advocacy and social justice concerns. Transmission of

information intended to educate patients, staff and others about

topics essential to health care and well being, from simple “show

and tell”, to a more formalized teaching in-service.


Management / Organization


Excellent time management and prioritization abilities. Promotes

cooperation among staff and others. Provides direction to and

acts in an advisory role to field staff, and the clinical support team,

as it relates to achieving desired patient outcomes. Collaborates

with clinical coordinators and clinical managers by sharing

information related to staff competences and cooperation.


Communications Skills

Efficient and effective oral, written and computer communication

skills to exchange and transmit information to the field staff,

physicians, families, patients, residents, payers, and other stake

holders. Facilitates interdisciplinary team communication

when necessary to achieve established goals and outcomes.
