We are seeking a full-time RN for our Hospice Program that will be based at Porter Hospice in Centennial and serve as the main on-call nurse for our home hospice program between 11:30 pm and 8:30 am.  While on-call, this nurse will go out on Death Visits and other crisis calls through the Denver Metro area between 12am and 8 am.  While not on visits, responsible for administrative duties (quality review of Hospice Plan of care, MTH tasks) and back up clinically for Porter Hospice as needed. 



 



Assumes responsibility and accountability for facilitating, communicating, and collaborating with both the healthcare team,



and the patient/family to identify and meet the physical, emotional and spiritual needs of the patient. Promotes the optimal



health, well being and safety of the patient through use of the nursing process and in accordance with patient care



standards, guidelines and the State Nurse Practice Act. Demonstrates personal accountability for relationship-based care,



organizational mission, and core values.



 



Minimum Education Requirements



Graduate of an accredited School of Nursing.



Bachelors of Nursing (BSN) Preferred



 



Minimum Experience Requirements



One year experience preferred.



Nursing skills and knowledge base specific to specialty.



 



License/Certifications



Current Colorado Registered Nurse (RN) license/ Current RN license from a participating state in National Licensure



Compact;



Current Basic Life Support (BLS) certification from American Heart Association required;



Current Colorado Driver's License for field staff;
