Job Summary:

Shares ownership with the attending physician and unit manager to expedite patients correct level of care, services and financial considerations for the appropriate length of stay. Collaborates daily with the clinical care team to expedite the patients care analyzing cost, quality and utilization to facilitate the admission, continued stay and discharge of the patient by combining the objectives of established clinical pathways and utilization management standards.

This position provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal of the position is to facilitate and promote interdisciplinary continuity of care and cost effectiveness through the integration and functions of case management and utilization of resources.

Job Requirements:

  • Education: Bachelor's Degree in Nursing or health-related field OR fully Licensed MSW (LMSW) required.
  • Experience: Two years previous discharge planning and/or directly related Case Management experience within the last 5 years required.
  • licensure: Case management certification preferred (CCM and/or CHCQM)
  • Skills and Abilities: Knowledge of InterQual criteria preferred. Clinical experience in the assigned area preferred. Knowledge of community agencies for post acute support, (medical and/or financial) preferred. Interpersonal skills necessary to negotiate with staff, families, patients and physicians. Excellent communication (oral and written) skills to insure patient, family and team satisfaction. Ability to work independently, setting priorities to coordinate the patient throughout their assigned unit.
