Utilization Review/Discharge Planning, Case Managers
Full-time day shift and Full-time evening shift
Screen patients for acute hospital admission and continued stay.
Utilization review and communication with team in the Emergency Room regarding in-patient Medicare patients and readmissions.
Review one day and short stays, close follow-up with initial reviews and denials.
Provide retrospective review for short stay.
Communication with admissions, patient investigation on transferred patients to ensure authorization was provided.
Promote a safe and timely patient flow through from admission to discharge.
Collaborate with the interdisciplinary team to coordinate appropriate discharge plan.
Coordinate patient and family education for discharge with the educator and interdisciplinary team members.
Data collection and analysis.
Licensure in NY State., Three years experience in acute care hospital, Good writing skills, computer efficient in Microsoft, Excel, and PowerPoint. Excellent communication and interpersonal skills. High level customer service skills. Flexibility, professional and courteous. Works independently. PRI Certified required
Experience in CM, UM, Quality or Coding. Certification in CM, UM, Quality, Risk, Coding or Milliman preferred. Working knowledge of Milliman or Interqual preferred. Knowledge of regulatory requirements preferred.
