The position is an exempt position with hours of 8:00-5:00 with flexibility needed to occasionally start early/stay late as needed.

Responsibilities:
In collaboration with the Medical Home team, the inpatient transition coordinators and community healthcare partners, the RN Care Coordinator is responsible for direct and indirect patient care across the care continuum, with an emphasis on coordination of care for designated risk stratified populations. The RN Care Coordinator will work with providers, patients and their families to establish patient centered goals and a plan of care to support achievement of goals, working with local and central care team members as well as additional external customers. Other duties as assigned.

Summary:
A minimum of 4 years of experience as a Registered Nurse is required.

Experience in Medical Surgical or primary care background is preferred. Previous experience as a case manager is preferred.

Professional knowledge of nursing theory and practice at a level normally acquired through completion of a minimum of two years education at an accredited School of Nursing in order to be eligible for licensure as a Registered Nurse is required.

Bachelor's degree in Nursing is preferred.

Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required upon hire.

Self-directed and provides input on estimating timelines, work effort and documentation. Provide regular progress reports. Demonstrate effective verbal and written communication skills. A high degree of positive energy and the ability to rapidly initiate positive relationships with others to develop trust. Demonstrates creative solution based problem solving skills and a commitment to placing the team's needs first with a willingness to accept and adapt to change. Demonstrate effective conflict management skills. Keyboard skills and experience working with Electronic Medical records and Microsoft office required.
