Primary function is to administer skilled Occupational Therapy for clients of all ages in their place of residence and coordinate care with the interdisciplinary team, patient/family, referring agency, care planning team, and OASIS as necessary.
Perform clinical assessments and identify patient needs.
Implement / develop / revise and document the plan of care.
Ensure quality and continuity of care based on evaluation and ongoing assessment.
Evaluate patient occupational performance in the areas of self-care, work, leisure, and rest.
Supervise and provide clinical directions to the COTA.
Effectively communicate with patient, family, and health care professionals.
Graduate of an accredited school of Occupational Therapy
Current OT license for the state of TX
Home Health experience
Valid drivers license
Current auto liability insurance.
