Seeking a Nurse Practitioner for the home Heart Success program. The qualified candidate will have a BSN and Masters degree and a current NJ nursing license. BLS certification and at least 5 years of acute care Cardiac / Telemetry experience is required. The Nurse Practitioner will be responsible for the oversight of the agencies telehealth and Heart Failure programs. Additional responsibilities include collaboration with AHS Heart Success team, as well as, system cardiologists to care for complex heart failure patients in the community; provide education for agency staff on management of Heart Failure in the community; provide CEUs and in-service programs as requested for agency staff and potential referral sources; and education and mentoring of telehealth staff in management of complex cardiac patients. The position requires a Nurse Practitioner or Clinical Nurse Specialist with a background in Heart Failure management. Candidate will also have the ability to utilize the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served, through the model of Shared Governance, collaborate with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families, demonstrate and embrace the PRIDE core values (Professionalism, Respect, Involvement, Dignity and Excellence) in every aspect of their job, and have excellent communication and computer skills.
