Job Summary:
The Patient Care Coordinator (PCC) is a registered Nurse responsible for coordinating healthcare services across the continuum of care by monitoring, evaluating and modifying the treatment plan to achieve optimal patient outcomes under the direction of the Director of Case Management and Nursing Services. The PCC employs critical thinking to integrate objective data with knowledge gained from an initial interview and ongoing assessment including the objective and subjective experiences and healthcare history of the patients. The PCC will use this critical thinking process to apply the best evidence based practice and research to interdisciplinary care coordination from admission to discharge planning. The PCC will continually evaluate quality and effectiveness of coordinated patient care practice and seek to optimize outcomes for an appropriate and safe discharge for patients. Medical/clinical knowledge, technical skills, clinical judgment, interpersonal skills and communication skills as indicated in the current Comprehensive Accreditation Manual for Hospitals are essential components of nursing and case management practice. Additional professional organizations may be referenced when appropriate to guide practice and standards within the work place.

This position is represented by CNA and is covered by the terms and conditions of the applicable collective bargaining agreement.

Minimum - Three (3) years acute hospital clinical experience. For critical care areas, two of these years within the last five must be in ICU, IMC, or COU as a Registered Nurse, or, successful completion of the St. Rose critical care course within 180 days of hire if critical care experience is more than five years old. In addition One (1) year of acute care related Case Management experience and/or one (1) year or greater current (within 5 years) experience as charge nurse, relief charge nurse, or nursing management is required.
Preferred - Certification in related clinical specialty area.

Minimum - Bachelor of Science in Nursing (BSN) or health related field. Current St. Rose employed Case Managers and relief/Charge Nurses that are selected will be given three (3) years to complete BSN.

Knowledge of all aspects of documentation in Midas, CERNER, SMS, and the AWARIX system supporting patient flow and continued care management. Basic understanding of desk top computers and work related software. Demonstrates proficiency in nursing process, problem solving and the performance improvement process. Maintains current knowledge in basic health care economics, trends, and reimbursement methodologies and applies knowledge to daily activities.

Special Skills
Meets all of the unit based competencies annually. Within 90 days of hire, must have a strong working knowledge of all aspects of documentation in Midas, CERME, CIS, SMS and the AWARIX system supporting patient flow and continued care management. Basic understanding of desktop computers and work related software. Demonstrated proficiency in nursing process, problem solving and the performance improvement process. Successful completion of Interqual training annually. Demonstrated organizational, collaboration, negotiation and leadership ability. Strong customer service skills and the ability to handle highly confidential information. Demonstrated ability to understand the unique infant, child adolescent, adult and geriatric population and effectively meet those needs relative to growth, assessment and development of those patients. Demonstrated ability to coach/mentor and role model employees on the unique care of the patient populations in their clinical service area. Monitors the clinical care of patients and provides clinical support to improve patient care, staff satisfaction, patient satisfaction and patient outcomes. Coordinates clinical resources to achieve Clinical Quality Outcomes: Including but not limited to Core Measures, NPSGs, Stroke Measures of Success, SCIP, and HCAHPS.

RN= current license as a Registered Nurse in the State of Nevada
CPRBLS= Basic Life Support Certificate.
ACLS=Advanced Life Support Certificate required for critical care settings.
Oncology Nurse Specialty for Certification in Chemotherapy required within 180 days of hire for Oncology setting.
Preferred: Specialty certifications as applicable.
