Overview:Responsible for Senior BuenaCare PACE participants assigned to the team. This position works closely with providers and nurses to coordinate services and advocate for participants. Responsibilities include, but are not limited to;  initial assessment, quarterly notes, semi annual re-assessments and annual re-assessment; conducting family conferences, assisting with community referrals and regular contacts with participants.
Responsibilities:- Responsible for performing initial and semi-annual assessments, quarterly notes and develops care plans for assigned team/participants and completes within monthly timeframe set by state guidelines. - Interview and screen participant’s family members and caregivers to indentify additional need for psychotherapeutic intervention and inform immediate supervisor. - Provide information and referral to participants and family and assist with coordination of services through public, private and social services agencies. - Act as a liaison with participant’s family, caregiver and for with other community agencies providing services to participants. - Document all PACE participants and family contacts in accordance with PACE documentation guidelines. - Works with PACE Care Coordination Team (RN, OT, PT, Activities) by taking lead in investigating non-clinical psycho-social problems as identified by the PACE inter-disciplinary team and documents in PACE Nextgen & PACE support systems. - Coordinates family conferences and attends team meetings with provider and R.N. - Perform participant psychosocial tasks as assigned by Social Worker Supervisor and PACE Administration. - Participates in daily PACE Inter Disciplinary Team meetings and other mandatory meetings. - Mandatory contacts with participants and develops and conducts specialized therapeutic groups for PACE participants at least one time a month. - Provide home visits if unable to contact by phone and provide feedback to team and supervisor. - Support and maintain a caseload as mandated by Title XXII requirements. - Perform other related duties as assigned.
Skills and Abilities:- Excellent customer service skills and ability to resolve basic/difficult customer service issues and exercise conflict management. - Ability to read, write and speak in a clear, accurate and professional manner; includes active listening skills and understanding medical terminology. - Ability to complete basic math computations. - Proficient in beginning/intermediate computer skills and typing. - Excellent follow through skills and attention to detail. - Strong quantitative, organizational, facilitative, oral and written communication skills.  - Ability to organize and prioritize goals and responsibilities. - Must be self motivated, organized, flexible and adaptable. - Excellent customer service; ability to be an effective communicator. - Thorough, detail oriented, able to follow through and work semi-independent. - Knowledge of DHCS, OSHA and safety regulations. - Knowledge of HIPAA Guidelines including the PHI privacy requirements.
Qualifications:- Bachelors’ degree in Social Work from an accredited university required. - A minimum of one year experience in case management/social advocacy with geriatric population required. - CPR/First Aid certificate required. - Bilingual English/Spanish, English/Tagalog or another second language strongly preferred.
