Job Description

Summary:

The Licensed Vocational Nurse II - TNH provides basic bedside care to geriatric patients under the direction of a registered nurse or physician, functioning within scope of license.

Responsibilities:

  • Contributes to the assessment of patients by collecting data for analysis by a Registered Nurse (RN).
  • Assists with Administering medications and observes patients for adverse reactions to medications or treatments.
  • Assists with performing routine laboratory tests and simple therapeutic services.
  • Assists with educating patients/families concerning disease, treatment plan including self care post discharge, holistic health needs, resources available to patient/family, rehabilitation and follow-up care.
  • Maintains clinical expertise and competency (to include population specific) as well as demonstrates thorough knowledge of procedures to deliver quality care in specialized situations.
  • Coordinates and delegates the delivery of patient care provided by selected members of the health care team and Advocates to ensure patient rights and preferences are integrated into care provision.
  • Supports professional standards of care delivery by:
    • (a) functioning within the scope of LVN title act and Network policy and procedures
    • (b) assuming responsibility for own learning needs
    • (c) utilizing appropriate resources (RN and other healthcare members) as needed.
  • Functions as an effective team member with the healthcare team, patients and families using basic communication and team building skills and promotes teamwork by assisting others with patient care and department activities, promotes cleanliness and a safe environment according to the network EOC guidelines, performs all duties in a manner that will assist in the achievement of the Network Plan and Goals.
  • Performs other duties as assigned.
  • May be required to be "on-call."

 Education & Experience:

  • 1 year of nursing practice in a Long Term Care (LTC) Facility is preferred.
  • High school diploma or equivalent is required.

Licenses & Certifications:

  • Basic Life Support, current CPR certification from the American Heart Association is required.
  • Current Licensed Vocational Nurse (LVN) license to practice in the state of Texas is required or must be obtained prior to hire or job transfer date.
