Job Description:
  • Demonstrates a thorough knowledge of work practices/ responsibilities, both Case management and Social Work. Works with Director to ensure that staff is informed of regulatory changes and that all state, federal laws and regulatory agency requirements are met. Updates policies and procedures as needed.
  • Participates in hospital committees and administrative assignments (ie budget, staffing and other delegated responsibilities).
  • Educates, supervises and supports the staff on Case Management related issues. Assists the Director in identifying issues and intervenes to achiever positive and timely outcomes by observing staff performance, actively participating in patient related interventions with focus on timely and efficient discharge. Shares observations with Director and staff.
  • Works with the Director and Physician Advisor to implement and maintain the Care Coordination reviews and meetings. Educates and trains Case management staff on the focus of the rounds and documentation of the meetings in the patient medical record.
  • Monitor the web based programs such as eRequest and HPF for completion of medical records and completion of reviews into the EQ system for payment. Assign cases to staff to complete task. Work with HIM/Shared Services to complete incomplete records in regards to correct orders/status.
  • Act as an additional system administrator for EQ health (Medicaid) to add or delete staff access as needed. Assist staff with entering data in the EQ system.
  • Ensure that all department staff achieves compliance with mandatory education requirements. Provide mandatory education to staff as needed. Provide departmental education to staff ie Meditech, Midas.
  • Monitor referrals to E H R for secondary review of 1 day stays and assist staff with the referral process. Review monthly invoices from E H R and submit to AP for processing and payment.
  • Work with Director, Physician Advisor and Nurse Case Managers with referrals to insurance carriers concerning patients™ status and completion. Notification of appropriate departments for changes in status.


Qualifications

Requirements:
  • Must have FL RN License
  • Bachelor Degree required
  • Must have Master's Degree or actively pursuing Master's Degree
  • 2 years of Case Management experience required
  • 2 years of clinical nursing experience required
  • Must have current BLS
