The Home Care Liaison for Skilled Nursing Facilities is a licensed nurse who has a thorough understanding of available home care services and products, eligibility requirements, and resources available in the community. The Home Care Coordinator acts as a resource to skilled nursing facilities, and facilitates continuity of care for the patient referred to receive home care services. The Liaison assesses patients, collaborates with the health team and physicians, and prepares the hand-off packet to transition the patient to home care services. The Liaison is actively involved in promoting services and the presentation of educational programs to promote the JHM continuum of care.
To qualify, you must have an LPN. RN is preferred.
