Progessive social service agency is seeking a motivated, detail-minded Registered Nurse to help coordinate medical services for adults with intellectual/developmental disabilities living in beautiful group homes in Chicago and the north/northwestern suburbs. We are looking for someone that can work with a team with our medical services department at our Mount Prospect clinic, but will also have flexiblity to travel to some of the group homes to provide training to staff. Clinical skills and medical case management skills helpful. Experience with persons with disabilities and applicable training preferred. Computer proficiency required.Full-time, every 3rd weekend on call. 

  • Advocate for consumer rights and services,

  • Assist with the scheduling of appointments and follow-up treatments and consultations, attending such when the level of complexity indicates, 

  • Assist with the ordering, receipt, and distribution of medications as ordered by physicians,

  • Assist with the collection of documentation of medical care provided to consumers, participating in employee retraining and disciplinary action as warranted,

  • Monitor medications and treatments for side effects, toxicity, therapeutic results and provide coordination for follow-up as needed,

  • Participate on interdisciplinary teams to develop transition plans and behavioral health plans as needed,

  • Ensure that all staff assisting with medications are fully trained according to DHS Rule 116 guidelines,

  • Complete annual nursing assessments and physicals according to schedule as assigned,

  • Prepare medical summaries for the interdisciplinary team as assigned,

  • Assist with the preparation and distribution of the medical appointment calendar in the Harmony system,

  • Assist with the accurate and timely entry of all medical events in the Harmony system,

  • Complete monthly and quarterly medical reports in an accurate and timely manner,

  • Complete incident reports according to policy,

  • Participate in week-end on-call rotation,

  • Other duties as assigned.
