The Palliative Care Coordinator is responsible for planning, developing, implementing, coordinating and evaluating palliative care services. Included in this responsibility is developing and maintaining strong relationships with physicians and hospital staff to deliver palliative and comfort care to seriously ill and dying patients. This position is accountable for those functions which enhance the delivery of palliative care through such activities as education, standards and program development, quality improvement activities, consultation and case management. The coordinator is responsible for promoting high quality, cost effective care for palliative care patients.


Also, providing care, education and emotional support to patients with chronic and terminal illnesses and their families.


Minimum 1 year acute care experience and hospice experience preferred.


Education level necessary to obtain required licensure and certifications


Current ELNEC training preferred required within one year of employment.


Current California Registered Nurses license


Current BLS certification, AHA required.
