About This Opportunity:
Glendale Memorial seeks an experienced clinician to supervise the care of our stroke patients in our renowned Heart Center. This key team member performs duties in alignment with the mission and policies of the organization. They will also perform duties in support of and in compliance with the performance improvement plan, TJC, and other licensing/accreditation and regulatory agencies. Stroke Coordinator provides: Direct patient advice, guidance, education, referral and follow-up for stroke center patients; serves as the patient's personal health assistant and is the primary contact for questions, coordination of care, and navigation through a stroke diagnosis; oversees quality measurements, continuous quality improvement and all patient satisfaction initiatives on topics of Stroke; outreach to community groups, partners, and physicians on issues of stroke; program development and long term planning, working with marketing and strategy as needed; The Stroke Coordinator is expected to work closely with staff and physicians to create seamless coordination of care, and be a key participant in service line development.
Minimum 5 years acute care experience as an RN. Prefer critical care, telemetry, neurology experience. Experience in Patient Care Coordination preferred. Bachelor's Degree required Current BCLS: ACLS. Basic computer skills: Microsoft Word, Excel, Powerpoint. NIHSS Certification. Armenian speaking preferred Current California license as a Registered Nurse
