Overview:







The Licensed Practical Nurse is responsible for administering and providing care to patients and families according to the nursing plan of care for terminally ill patients admitted to the VITAS program.  The LPN/LVN will provide care in accordance with the Nurse Practice Act in the state where the VITAS program is located.

 

  • Teaches families and other primary care persons appropriate care techniques for the patient.
  • Provides accurate documentation with visit itineraries while providing direct skilled bedside nursing care.
  • Evaluates patient/family needs in the preparation and execution of the interdisciplinary plan of care for each patient on the team.
  • Evaluates effectiveness of treatments and monitors patients' reactions.
  • Maintains a clean and safe environment. 
  • Maintains accurate clinical notes on interdisciplinary charts.
  • Reports significant findings or changes of the patients' condition to the Registered nurse, Team Manager or Physician.
  • Documents reports in clinical notes.
  • Participates in Nurse LPN/LVN on- call rotation/schedule in order to meet the needs of patients and families.

 

This is a per-diem LVN position providing patient care throughout Orange County on the Intensive Comfort Care team.  The per-diem LVN will work various days throughout the week and weekends from 4pm-12am or 12am-8am as needed.

Qualifications:







  • Qualified candidates must be currently licensed as a Licensed Practical Nurse.
  • Must possess at least one (1) year, preferably two (2) years of satisfactory nursing experience working as an LVN. 
  • At least one (1) year of which LPN/LVN has worked in a hospital on an adult medical-surgical or similar unit is strongly preferred.
Education:







  • Completion of course of study to acquire LPN/LVN state licensure required.
  • Licensed to practice in the State of California required.
