This position is a Registered Nurse who is responsible for ensuring that patients are receiving care management services across the continuum of care, in multiple settings, while addressing the ongoing individual needs of each patient. Provides supervision to Care Management personnel; contributes to the hiring and termination decisions; develops job descriptions, competency and performance standards; monitors assignments and productivity. The Manager serves as a departmental role model and resource to patients as well as internal and external customers in order to assure continuity of care and optimal clinical and financial outcomes. The Manager reports directly to the Director, and has direct oversight for Care Management functions. The role provides counseling and training, addresses disciplinary problems, and appraises performance.
