Manager of Case Management & Social Work Department. Direct, plan and supervise activities of the case managers and social workers to coordinate continuity of care, optimize appropriate utilization of services, and ensure compliance with Mercy, Centura and regulatory requirements. Serve as Mercy’s representative to and liaison for Centura’s Case Management and Social Worker initiatives. Coordinate oversight, implementation and successful compliance with Centura and Mercy initiatives. Serve as co-chair of Utilization Review Committee. Areas of focus consist of case management, social work, team management, reduction of high utilizers of the ED and hospital, Recovery Audit reviews and appeals, and patient status certification compliance. Serve as liaison with medical staff, nursing, nursing administration, ancillary departments and senior administration. Facilitate representation of the hospital with federal review agencies, community resources, and insurance companies. Responsible for budgeting, staffing and planning for case managers and social workers.
BSN preferred however additional experience will be accepted in lieu of the BSN requirement
2-3 years supervisory experience
Preferred: 2 3 years of experience in care management to include discharge planning and utilization review.
RN license valid in the State of Colorado
