Responsible for planning, coordinating, managing, and evaluating activities of a multidisciplinary team to ensure delivery of greater home care services to patients. Oversees scheduling and assigning patients to nursing staff to achieve maximum productivity and cost effectiveness standards. Knowledgeable of standards of practice, positions, procedures and regulations appropriate to home care. Responsible for implementing patient services that maximize third party reimbursement while monitoring provisions of charity care in accordance with Agency policies and procedures. Provides home care visits, when necessary, and submits corresponding documentation regarding patient and each visit.home care, nursing, RN, homecare, nurse, home care nurse, home care RN
BSN degree from an accredited school of Nursing. Master's degree preferred. Two years experience in a Certified Home Health Agency or Long-Term Care. One year supervisory experience preferred. Minimum one year experience in a specialty required. Proficiency in Microsoft Word and Excel. NYS RN license required. Excellent customer service skills. Ability to work with an integrated healthcare team. Demonstrates critical thinking and problem solving skills. Excellent written and verbal skills. Proficiency in interviewing and physical assessment skills. Strong leadership and management skills.
