We have an exciting opportunity to join our team as a Manager of Clinical Care Coordination. This position is responsible for managing care coordination across the continuum of care for the population management activities of the Network Integration Department. Nurses in the Network Integration Department provide ambulatory care coordination services to patients within NYULMC's clinically integrated physician network. The position is accountable for the day-to-day direct management and oversight of nurse Clinical Care Coordinators, and for the performance of duties inherent in a managerial role. A focus of the job is the care coordination activities as part of accountable care contracts with health insurance plans.To qualify you must have Bachelor's degree in nursing, Master's degree, New York State Registered Nurse License with current registration, at least 3 years of clinical experience in acute care nursing and care management/coordination experience for a health plan, 5 or more years of supervisory, management or comparable experience in a related setting. The successful candidate will be knowledgeable of health insurance plans including Medicare and managed care requirements. This position requires someone who has good general understanding of health care, is self- directed, and demonstrates excellent communication and team building skills. Proficiency in Microsoft Office Word, Excel and PowerPoint is required.
