Job Summary:The Clinical Nurse Navigator will serve as a clinical resource with expertise in endocrine conditions, especially thyroid cancer. Prior knowledge of this topic is not required, although helpful. The Navigator will serve as a point of contact for referring physicians, patients, caregivers for patients diagnosed with thyroid cancer. He or she will function and collaborate with multidisciplinary teams as a coordinator of care, advocate, interpreter, educator and counselor for patients with the diagnosis of thyroid cancer. He or she will facilitate patient appointments, including laboratory, nuclear medicine, diagnostic procedures, and specialty physicians.

He/she will serve as the liaison throughout the facility and in the community regarding services provided for this unique patient population. He/she will facilitate patient appointments, including those made with labs, diagnostic areas and specialty physicians. The Clinical Nurse Navigator participates with patients, families, the interdisciplinary team and community resources to provide well coordinated, timely, compassionate, exemplary and interdisciplinary care. Serves as an essential link between patients and all other care givers.

The nurse should have the ability to access and interpret patient care information and have evidence of competency in planning and implementing individualized care plans. The ideal candidate would be self-motivated with a strong interest in personal and professional growth. 60% of the individual's time will be solely devoted to the coordination of thyroid cancer patients. 40% of the individual's time will be devoted to patient care. Their tasks may include t not limited to:


• Obtaining prior authorization for treatments
• Instructing patients on thyroid cancer, providing appropriate information
• Preparing charts for referral, readying patients for care with other providers (both inside and outside the institution)
• Ability to travel to clinical locations in the Baltimore metropolitan area
• Coordinating and scheduling patient for appointments and procedures
• Answering patient phone calls and directing questions to be appropriate location
• Participation in care conferences including tumor board
• Maintain a database including clinical information, demographics and treatment information for research purposes
• Retrieving medications or supplies from secured areas.
• Other tasks may include:
o answering telephones
o filling out disability forms
o Preparing charts for clinic visits
o Escorting patients to exam rooms
o Organizing patient charts per doctors organization guidelines
o Ordering medical supplies
o Other related duties as directed by the operations manager

Requirements:

Education:  Must possess RN degree, BSN preferred

2-5 YEARS of clinical RN experience as well as knowledge of physician practice operations and understanding of basic computer operations. Strong organization skills required. Endocrinology experience preferred.  Must be able to work in a high volume busy medical practice, ability to work well in a stressful environment.
