Under the general supervision of the Case Manager and Social Worker, supports the case management outcome model through clerical and support services in order to assist with the implementation of appropriate patient transitions. Accurately arranges services for assigned patient population including but not limited to: Home Health, Durable Medical Equipment, Transportation, Housing, Skilled Nursing Facility and other Placements, Community Resources in order to assure appropriate continuing care services are in place for patient transitions. Supports payor documentation through the provision of information to payors, documentation of authorizations and payor clinical information requests. Other duties as assigned.
Minimum of two years of experience in a healthcare related field is required.
5 years of experience in a healthcare related field is preferred. Hospital experience is preferred.
High School diploma or equivalent is required.
Associate's or Bachelor's degree in a healthcare related field is preferred.
Medical terminology, insurance, and customer service is required
Analytical skills necessary to obtain information from patients and families, external customers, and ability to interact professionally with patients, families, external customers, and hospital personnel. Good organizational skills to enable management of multiple simultaneous tasks. Excellent communication skills. Knowledge of Microsoft Office products.
