Our groundbreaking hospice and palliative care programs offer a significant difference when dealing with a life-limiting condition. We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.


Working with a collaborating physician within the health plan, provides and coordinates primary care services to an assigned group of Medicare Advantage health plan members residing at home. Performs clinical assessments in the home, develops care plans and insures access to preventive care services in order to meet member and plan health outcome objectives. Works with health plan care managers to schedule and follow-up home visits and to update the care plan.

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program required.
  • NYS RN/NP or PA License, Registration, DEA certificates.
  • One to two years clinical experience in role of nurse practitioner or physician assistant required including 1 year in a geriatric specialty.
  • Previous clinical experience in acute, subacute and/or long term care setting preferred.
  • Prior experience in managed care a plus.
  • Proficiency in computer skills, electronic medical record, case management software, word, excel required.
