The Licensed Practical Nurse administers nursing care according to the plan of care to terminally ill patients admitted to the VITAS  program, in accordance with the Nurse Practice Act in the State of California.

 

  • Provides direct skilled bedside nursing care.
  • Evaluates patient /family needs in the preparation and executions of the interdisciplinary plan of care for each patient on the team and the effectiveness of treatments and monitors patients reactions.
  • Performs selected acts in care of the terminally ill under the direct supervision of a registered nurse; such acts include the administration of treatments and medications.
  • Participates in Nurse LVN/LPN on- call rotation/schedule in order to meet the needs of patients and families.

 

This is a full-time Continuous Care LVN position providing patient care mainly in Eastern Contra Costa and San Joaquin Counties.  This position reports out of the main office located in Walnut Creek, CA.

Qualifications:
  • Must possess at least one (1) year working experience as a LVN.
  • Prefer two (2) years satisfactory LVN working experience with at least one (1) year working on an adult medical surgical unit in the hospital.
  • Currently licensed as a Licensed Practical Nurse in the State of California required.
  • This is a per-diem position requiring the LVN to have a flexible schedule with the ability to work various days/hours throughout the week and on weekends as needed.
  • This position is based out of the main office located in Walnut Creek, CA.
  • Patient coverage includes Northern Alameda and Contra Costa Counties.  Qualified candidates will reside in one of these counties in order to respond to patient/family needs in a timely manner.
  • Valid driver license with appropriate insurance required.  Mileage reimbursement provided.

 

Education:
  • Completion of course of study as required to acquire LVN licensure in the State of California.
  • Graduate of an approved school of Practical/Vocational nursing.
