Responsibilities:
Coordinates care transition for patients identified as high risk for avoidable re-admission. Responsibilities include initial inpatient contact and immediate post discharge communication and collaboration with patient and community resources such as primary care physician, home health care, skilled nursing facilities, and Medical Home Care coordinators. Actively involved in management of metrics related to readmission rates and identification of high risk populations. Follows the ANA Standards of Nursing for Patient Care. Other duties as assigned.

Summary:
A minimum of 3-5 years medical/surgical experience is required.

Experience in Older Adult/Geriatric Discharge Planning is preferred.

Bachelor's degree is Nursing is required.

Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact) is required.
