I. General Summary
Independently provides and coordinates care provided to patients within a clinical specialty. Serves as a fully effective and participative member of the Cardiac OR Team. Demonstrates and models the full range of nursing professional practice in both care provided and in participation in development of strategies to improve patient outcomes, satisfaction with service and cost effectiveness at the practice setting level. Is accountable to patients and families for the processes and outcomes of care during an episode of care. Is accountable to peers for the advancement of group practice on the practice setting, and to the nurse manager for the totality of work performance.
II. Principal Responsibilities and Tasks

Clinical Practice/Care Delivery
1. Competent in care of practice setting specific patient population; independently practices with solid knowledge and clinical skills; engaged in advancing own practice 2. Serves as Clinical Resource for others 3. Utilizes and integrates evidence into practice 4. Models high level of clinical competency and expertise related to patient populations in areas of assignment. Participates in developing/revising standards of practice to meet changing patient population requirements 5. Serves as a resource in planning care for complex patients. Demonstrates knowledge of the principles of growth and development over the life span and possesses the ability to assess data reflective of the patient's requirements relative to his or her population-specific and age specific needs. Patient Care Management
1. Identifies the need for multidisciplinary services; initiates and tracks referrals as appropriate. 2. Takes the lead within the multidisciplinary team in monitoring patient progress, identifying problems and facilitating changes in treatment approach to meet patient care objectives. 3. Identifies ways to improve the discharge planning process to reduce length of stay and prevent delays in discharge. Monitors patient progress towards discharge and takes the lead with colleagues in facilitating timely discharge. 4. Actively participates in the development and implementation of clinical paths. Oversees practice setting level implementation for selected paths. 5. Models highly effective interpersonal and communication skills. Responds effectively to conflictual or complex issues in coordinating services on behalf of the patient/family. 6. Monitors competency, documentation and outcomes and counsels non-licensed staff regarding performance. Identifies ways to facilitate working relationships among all levels of staff to meet patient care requirements. 7. Effectively sets priorities for patient care and practice setting needs. Coaches and assists staff to manage time effectively. 8. Takes a leadership role in incorporating diverse payor requirements into patient care processes at the practice setting level. Actively participates in and takes the lead in professional practice partnership activities at the practice setting level and beyond. Takes personal responsibility for group outcomes. Provides feedback to peers and manager about effectiveness of partnership activities. 2. Demonstrates responsiveness to practice setting needs. Coordinates and facilitates scheduling, assignments and governance activities, based on changing patient care requirements and mutual negotiation with peers. 3. Participates in activities to improve practice setting level systems for performance management. Holds colleagues accountable, and demonstrates accountability to colleagues. 7. Follows up and supports efforts of colleagues to implement improvement action plans. 3. Participates in implementation efforts addressing Nursing Sensitive Quality Indicators (NSQI) 4. Evaluates and provides feedback to other staff regarding precepting skills. Evaluates and improves the practice setting level orientation/preceptorship program. 4. 1. Licensure as a Registered Nurse in the state of Maryland, or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing, is required.
2. Minimum of two years of experience in a comparable setting. 3. Maintenance of 10 contact hours of education/year. 4. Member of a professional organization. 5. Demonstrated ability to act in a clinical leadership role. 6. Demonstrated ability to promote professionalism through acts such as involvement in professional organizations, teaching, research, publishing, and/or certification in area of specialty. 7. Demonstrated ability to lead others, such as effectiveness as charge nurse, preceptor, or clinical resource. Demonstrated effectiveness as a multidisciplinary team member. 8. Ability to learn and use computer systems in order to manage patient information. 9. Demonstrated/documented highly effective interpersonal and verbal and written communication skills.
