About the Opportunity

A healthcare services organization is currently seeking a Registered Nurse (RN) to join their staff as a Community Health Nurse to do home visits in Brooklyn, Manhattan, Queens, and Bronx. In this role, the Community Health Nurse will be responsible for providing skilled nursing care to patients in their home, including nursing assessments, nursing diagnosis and interventions. Assist in the supervision of Home Health Aides in the field.

Job Description

The Community Health Nurse:

  • Admits patients to the agency in accordance with agency admissions policy and regulatory requirements
  • Establishes and provides a plan of treatment for the patient based on results of the nursing assessment visit, client needs and physician orders
  • Involves the patient and appropriate others in the planning, implementation and assessment of the intervention/treatment program
  • Complies with Start of Care Assessment and obtains appropriate admission consents as per agency policy and regulatory requirements
  • Submits all admission paperwork within one week of the initial visit
  • Provides an initial verbal report to the Clinical Manager within twenty-four (24) hours of the initial visit
  • Provides education to patient/family and paraprofessional staff as appropriate, on plan of care, disease process, medications, universal precautions, infection control, nutrition, safety emergency measures, patients care and community resources
  • Provides written instructions and educational materials to the patient as indicated
  • Instructs, supervises, and evaluates Home Health Aides, Personal Care Aides and Housekeepers in the home setting on initial start and every sixty (60) days/ (90) days.  Informs Clinical Manager of inability to supervisor aides timely
  • Communicates changes in patient's condition, changes in treatment/meds, progress towards goals and response to treatment on weekly basis to the Clinical Manager
  • Plans for discharge of the patient taking into consideration goal achievement, and provides for appropriate follow-up or referral
  • Documents on an ongoing basis, services provided and response to services/treatment and changes in status relative to the plan of care
  • Submits weekly itinerary by Friday of previous week
Required Skills
  • 2+ years of relevant experience
  • Graduate of an accredited school of Nursing
  • NYSRegistered Nurse (RN) license
  • Patient oriented
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
  • Able to multitask efficiently and effectively

  • Bachelor's Degree (B.A.) in Nursing from a four-year college or university
