Responsibilities:
Responsible for the coordination of services for individuals with spinal cord injury (SCI) due to trauma or disease within the Spinal Cord Injury System of Care. The services are based on the interdisciplinary plan of care and are established in accordance with current policies and guidelines to ensure optimal patient outcomes. The System of Care includes acute hospitalization, inpatient rehabilitation, outpatient follow-up and addresses life long needs. Responsible for the coordination of comprehensive evaluations, annuals evaluations and potential self referrals from external facilities. Participates in Spinal Cord System of Care performance improvement activities. Other duties as assigned.

Summary:
A minimum of five years of acute care nursing experience is required.

A minimum of two years of clinical practice in designated specialty is preferred. Spinal Cord Experience is preferred. Prior utilization management or case management experience is preferred.

Professional knowledge of nursing theory and practice at a level normally acquired through completion of a minimum of two years education at an accredited School of Nursing in order to be eligible for licensure as a Registered Nurse is required.

Bachelor's degree in Nursing preferred.

Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 90 days.

Other information:
Analytical skills necessary to assess patient's discharge needs and develop solutions to problems concerning patients' mental status or physical well-being to ensure optimal patient outcomes. Interpersonal skills necessary to instruct and maintain effective contacts with a variety of hospital personnel.
