The Outreach Program Case Manager is responsible for patients who require special care coordination due to the complexity of their illness, home situation, or frequent use of emergency services or hospital admissions. The Outreach Case Manager will coordinate all ambulatory care needs. The Outreach Case Manager will perform an initial assessment, develop a plan of care, provide evidenced health coaching, refer the patient to appropriate resources according to assessed needs within the patients benefit structure utilizing community, state, federal resources as needed. The Outreach Case Manager will be the liaison between the Hospitalist, Primary Care Physicians (PCPs) and Health Plan's Disease Management and Case Management Programs and ancillary providers, maintaining contact with the member through all transitions of care. - CA RN or LVN license
- Minimum of 2 years case management, utilization management or discharge planning experience in an IPA, Health Plan or Hospital environment.
- Computer literate and able to use health information technology in care coordination.
