RN Coord_Patient Care
Under general administrative direction, coordinates referrals from physicians and health care facilities for patients; provides a significant level of patient education related to their illness and planned treatment. Responsible for guiding the patients with multiple DRGs through the complex care continuum. Assures patients receive care that consistently follows evidence-based standards and is in accordance with regulatory standards as defined by the regulatory agencies. Completes established competencies for the position within designated introductory period.
Nursing Diploma or Associate Degree in Nursing required. Bachelor of Science in Nursing (BSN) degree preferred.
Current RN license to practice in Texas. Nursing Diploma or Associate Degree in Nursing required. Bachelor of Science in Nursing (BSN) degree preferred. Three or more yrs clinical nursing experience in a hospital environment caring for high acuity patients required. Prior patient teaching experience preferred. American Heart Association BLS required. Advanced Cardiac Life Support (ACLS) required. EKG certification required. Must be detailed oriented and have the ability to work independently. Must also have the ability to work collaboratively with various administrative and clinical areas within the hospital. Must have excellent verbal and communication skills. Must be able to work well under pressure with frequent interruptions.
