Provides concrete social work services to adult and geriatric patients/families in St. Francis Hospital and Integrated Health Care System. The social work services are integrated into the functioning of the Case Management Department within the Division of Continuum of Care Management. The primary responsibility is to complete paperwork to transitions patients to Nursing facilities, including icompletion of PASRR paperwork, proactively assisting with the development and planning of complex discharges, and addressing transportation issues related to the discharge.All activities are guided by the philosophy and vision statement of Saint Francis Hospital and Medical Center. The position may require working at different campuses and outlying locations.
Education: Bachelor¿s in Social Work (BSW)
Experience: ne year of diversified, progressive experience in acute health care and/or other settings within the continuum.
Skills: This position requires a person with a positive attitude who is self directed, pleasant, cooperative, assertive yet compassionate displaying a professional demeanor with patients, families, physicians, and fellow employees. Must be organized, able to prioritize and balance competing tasks working with many different individuals. This position requires the knowledge of social work or nursing principles and practice and the ability to remain productive under stress; ability to adequately use or learn to use computer software and hardware applications. Basic knowledge of statistics, HCFA-OBRA regulations, ISDA criteria, JCAHO, levels of care in the continuum, and dynamics occurring in health care delivery systems a necessity. The ability to function in a fast-paced, integrated interdisciplinary model of Patient Focused Care to improve patient care and outcomes.
Hospital or LTC experience preferred
