Position Summary: Under the supervision of either the Center Manager or the transportation manager , responsible for providing and assisting participants of LIFE at Lourdes (LL) with activities of daily living, restorative and supportive care. Assists with organization and implementation of activity program. Monitors health status and provides a safe environment for participants. Escorts participants while being transported on LIFE vehicles. Also assumes responsibility for communication with center and transportation department. Functions as a member of Interdisciplinary Team on an as needed basis.
Education: HS/GED preferred.
Experience: 1-2 years working with the frail and elderly.
Current CNA Certification in State of New Jersey and/or Home Health Aide Certification
Knowledge, Skills and Abilities Required:
Skilled in establishing and maintaining effective working relationships with participants, co-workers and the public, knowledge in working with frail and elderly to support Activities of Daily Living, Skilled in identifying and recommending problem resolution, ability to work independently with minimum supervision, Ability to communicate clearly and effectively, Ability to work effective with culturally, economically and educationally diverse populations.
