Provides social work services to the Neonatal, Pediatric Adolescent, Adult and Geriatric patients/families/staff in the Saint Francis Hospital and Integrated Heath Care System. The social work services are integrated into the functioning of the Case Management Department within the Division of Continuum of Care Management. The scope of practice includes, but is not limited to diagnostic assessment, counseling, brief interventions in acute care, continuum based interventions in other settings, education, advocacy, support and solutions oriented focused interventions. All activities are guided by the philosophy and vision statement of Saint Francis Hospital and Medical Center. The position may require working at different campuses and outlying locations. The Code of Ethics (national Association of Social Workers), the Standards of Practice for Case Management (Case Management Society of America), and the Code of Ethics and Standards for Hospital Continuity of Care (American Association for continuity of Care) are incorporated in all policies and activities.
This is a per diem position
Education: Master's in Social Work.
Experience: Two to five years of diversified, progressive experience in acute care and/or other settings within the continuum.
Licensure: Licensed Clinical Social Worker (LCSW) preferred
Certification: Certification in Case Management (CCM)/Certification in Continuity of Care - Advanced (A-CCC) preferred.
Skills: This position requires a person with a positive attitude who is self directed, pleasant, cooperative, assertive yet compassionate displaying a professional demeanor with patients, families, physicians, and fellow employees. This position requires the knowledge of social work or nursing principles and practice and the ability to remain productive under stress; ability to adequately use or learn to use computer software and hardware applications. Basic knowledge of statistics, HCFA-OBRA regulations, ISDA criteria, JCAHO, levels of care in the continuum, and dynamics occurring in health care delivery systems a necessity. The ability to function in a fast-paced, integrated interdisciplinary model of Patient Focused Care to improve patient care and outcomes.
Hospital or LTC experience preferred
