  • Position Summary:The Care Transition Coordinator (RN) is responsible for the interdisciplinary planning, coordination, implementation and evaluation of transitional care services on identified populations.  This is a unique role that will work with inpatients and follow them once discharged, providing coaching and support to empower them to take charge of their own health.  The Coordinator’s time will be divided between inpatient visits, home visits, and telephone follow up.  Additional information on the Coleman model can be previewed at www.caretransitions.org


    Education:BSN required, Master’s degree preferred


    Experience:2-4 years of experience required, 5+ years preferred

     

    Certifications/Licenses: NJ RN license andCertified Case Manager preferred

     

    Knowledge, Skills, Abilities Required: Comprehensive knowledge of transitional care issues and processes; Homecare experience and/or Medical/Surgical experience; Excellent verbal, written , and computer communication skills; Negotiation, organization and teamwork skills;Coaching/teaching skills

    Knowledge, Skills, Abilities Preferred: Knowledge of evidence based guidelines for identified populations; Medicare/Medicaid guidelines; JCAHO guidelines (continuation of care)

     

     Physical Effort: Moderate physical effort (e.g. continuous typing, occasional lifting, prolonged standing/bending, etc.)
