As the program continues to expand, we are seeking multiple Patient Utilization Review Nurses to organize, coordinate, and provide care coordination and care management services to patients who are most at risk for health deterioration, sentinel events, and/or poor outcomes. Patient Utilization Review Nurses will also ensure that patients receive optimal and preventive care across multiple health settings and multiple physicians/providers.
