Assistant Director, Care Management
We have an exciting opportunity to join our team as an Assistant Director, Care Management.To qualify you must have a current NYS RN licenses with a Master's Degree in Nursing, Health Administration or related field. 5 years case, utilization management and discharge planning experience as well as 5+ years Supervisory experience with strong leadership skills. Excellent interpersonal, communication, negotiation and conflict management skills. Creative problem solving and clinical leadership. Change management, organizational and time management skills.
The successful candidate will assumes responsibility for providing leadership and management of clinical care coordination, utilization management, and discharge planning process. Assist the Director with systems, processes, and outcomes of clinical care teams and programs designed to address clinical, psychosocial, and financial needs of patients and families. Training, supervision and evaluation of team members, and participation in goal setting, program planning, workflow process monitoring, regulatory compliance, staff productivity, and assessment of good customer service, quality of work, outcome evaluation, and continuous quality improvement.
