The Clinical Documentation Specialist (CDS) will perform concurrent reviews of inpatient medical records to identify opportunities for documentation improvement. This position will generate written and verbal physician queries and functions as an expert source for information regarding the impact of documentation on patient’s clinical picture for quality outcomes measures, severity of illness, risk of mortality, and correct reimbursement. Responsibilities include, but are not limited to, the education of members of the patient-care team regarding documentation guidelines, including attending physicians, allied health practitioners, nursing, and Care Management. The CDS goal is to achieve a complete medical record by the time of patient discharge in order to facilitate and enhance the coding and DRG assignment process. The CDS functions as the liaison between clinicians (medical staff and allied health professionals), coding, Utilization Review, Compliance, Revenue Cycle, and the Patient Accounts teams. The CDS will facilitate performance improvement and utilization issues in relation to documentation of patient care, as well as work in conjunction with the HIM coders to ensure the accuracy and completeness of clinical information used for measuring and reporting physician and facility outcomes, as well as for ICD-10 education and training on how to properly document in compliance with third party, State and Federal regulations.
