|Job Description: |
The Clinical Program Manager for Heart Hospital Education is responsible to provide specialized nursing care to a select group of patients, and coordinate and manage major initiatives and clinical projects in the designated Clinical Program. The Clinical Program Manager role models the mission of the Clinical Specialty program at all times; to promote a culture of patient-centered care that emphasizes service and compassion. He/She interfaces directly with physician and clinical leaders of the Mount Sinai Medical Center, Departments Heads and all levels of staff
Nursing and/or Medical Director
Development, implementation, evaluation and overall management of the specified clinical
program and major initiatives that fulfill the mission of specified clinical programs
Participates in the development, implementation and coordination of clinical services and programs for designated patient populations. Activities may include but are not limited to:
Treatments and procedures
Patient and family education
Discharge planning and follow-up
Develops, implements and evaluates clinical initiatives.
Manages all phases of the project/program from inception to completion.
Manages the clinical and operational aspects of the clinical program
Incorporates an interdisciplinary approach to program implementation.
Incorporates principles of organizational change in program development,
implementation and evaluation..
Develops an evaluation method that is consistent with program goals, using knowledge of
statistics and research methodology.
Serves as a role model and clinical expert in practice.
Role models the values and attributes of The Mount Sinai Medical Center.
Demonstrates knowledge of project management.
Demonstrates knowledge of and commitment to service excellence; role models Mount
Sinai Service Standards.
Manages budgets specific to projects
Convenes and leads progress meetings and committee meetings.
Supports the mission, philosophy, goals and objectives of the Department of Nursing and The Mount Sinai Medical Center.
Is responsible and accountable for his/her own nursing practice.
Advocates the highest standards of nursing practice.
Approaches patients, families and other members of the health care team in a professional, respectful manner.
Serves as a clinical role model and resource to the nursing and clinical staff.
Serves as a professional resource to other members of the health care team.

presenter or author.
