Under the general supervision of the Manager, Home Care Services and according to established policies and procedures, the hospice registered nurse will case manage end of life care for the patient and family. As the Patient and Family Coordinator you will be a member of an interdisciplinary care team responsible for coordinating and implementing the plan of care from admission through discharge. This individual delegates and oversees the responsibilities of other licensed, certified, unlicensed staff and volunteers participating in the plan of care. Work is of a highly confidential nature. Performs other work of the program as required.
EDUCATION, TRAINING & EXPERIENCE
Graduation from an approved Nursing Education program with current NC or multistate RN licensure required with 1 year experience as a registered nurse including coordinating total patient care. The employee must have the ability to identify and accept the needs of people as they differ according to social status, race, beliefs, and their emotional, physical, and intellectual capabilities. Works effectively without direct supervision. Understanding of team concept and ability to work comfortably as an interdisciplinary team member. Valid NC driver’s license required. Annual TB test provided and required. CPR certification required and provided.
