RN Case Manager

St. Joseph Health System seeks an RN Case Manager focuses on the coordination of patient care across the continuum to assure quality outcomes, utilization/resource management, discharge planning and patient and family satisfaction. The Case Manager is an advocate for the patient and collaborates with medical, nursing and other health care disciplines to improve patient outcomes.
The successful candidate will possess the following qualifications:
• ADN; BSN is preferred
• Minimum of 3 years’ experience as a nurse in a clinical/hospital setting and at least 1 year of experience working in Case Management
• Active Texas RN license required
• Outstanding communication and people skills
