Full Time Emergency Department Case Manager
4:00 pm 12:00 am, with two days off
In this position, the incumbent will be collaborating with an interdisciplinary team to develop a plan of care which includes discharge planning, resource management needs, health education needs and the provision of information as it related to care management and the delivery of service care.
In order to be considered for this position you must have: NYS RN license and registration, Bachelors of Science in Nursing or Master’s degree (preferred) in Nursing or related health field. Must have five years (5)recent experience in adult Med/Surg, Emergency Room or Critical Care with a minimum of two (2) years of relevant clinical experience in case management or certification in case management (preferred)
