Position Requirements:New York State Licensed Registered Nurse, and BSN required. Certification in Oncology Nursing and MSN preferred. 3 years of clinical experience with at least 2 years of experience in an Oncology setting required. Completion of a Patient Navigation Course a plus, if hired must be completed within one year of employment. Strong Interpersonal skills as well as strong oral and written communication skills required.
Position Responsibilities:The Nurse Patient navigator serves as a consistent care coordinator, ensuring timely access, and seamless coordination of services through the continuum of breast care. The Patient Nurse Navigator will be an integral part of the health maintenance, teaching, counseling, collaborative planning, and restoration of optimal function of the patients' he/she serves. The position requires the Nurse navigator to facilitate care across the continuum of care including clinical judgement/decision making, professional relationships, and clinical leadership. The Patient Nurse Navigator will need to take into consideration the patient's stage of disease, and the individual needs following the diagnosis of breast cancer through end stage care if necessary.
