Job Summary: As a collaborative member of the clinic medical home team, this role works with designated primary care clinician(s) to care for patients in an enhanced RN role. This position is responsible for performing patient visits in conjunction with the physician, providing support for physician visits, engaging in patient education, patient triage, preventive health and chronic disease management within the scope of the RNs training and licensure and in collaboration with a comprehensive primary care team. Promotes communication, both internally and externally, to enhance the effectiveness of medical management services and is highly committed to team construct. Other duties as assigned.
Experience: A minimum of 2-4 years of experience as a Registered Nurse is required. Experience in Medical Surgical or primary care background is preferred. Previous experience as a case manager is preferred.
Education: Professional knowledge of nursing theory and practice at a level normally acquired through completion of a minimum of two years education at an accredited School of Nursing in order to be eligible for licensure as a Registered Nurse is required. Bachelor's degree in Nursing is preferred.
Special Skills: Essential to the success of this role is the ability to embody an attitude of service, a desire to learn and master new skills, exhibit a positive attitude, and demonstrate creative solution based problem solving and a commitment to placing the teams needs first. Keyboard skills and experience working with Electronic Medical records and Microsoft office required.
Licensure: Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required upon hire.
