Performs patient observation, collects data related to patient care, administers medication, treatment and procedures, implements the plan of nursing care and communicates findings to the RN according to policies and procedures while maintaining standards identified in the Colorado Nurse Practice Act. Assumes responsibility and accountability for facilitating, communicating and collaborating with the healthcare team to identify and meet the physical, psychosocial, functional, and emotional needs of the patients. Assume personal accountability for organizational mission, core and SHARE values.
POSITION DUTIES (ESSENTIAL FUNCTIONS, INCLUDE % OF TIME)
ASSESSMENT 30% Performs accurate observation & data collection. Appropriate communication of patient findings to the RN.
PATIENT CARE 30% With patient and significant others, contributes to & suggests resources in the formation and revision of POC. With patient and significant others, assists RN in development of discharge plan of care. Organizes time and priorities appropriately. Differentiates priority of risk & urgency.
CRITICAL THINKING 15% Uses critical thinking to implement plan of care. Communicates plan of care to patient and significant others and documents patient outcomes Directs staff in implementation of plan of care. Practices nursing in accordance with appropriate guidelines of practice.
QUALITY IMPROVEMENT 15% Continuously evaluates and assists team in revisions of the plan of care. Is accountable for the nursing process, responds to QI monitoring.
PERSONAL ACCOUNTABILITY 10% Attends staff meeting, including training meetings (70% or greater). Attained professional goals. Integrates new knowledge into daily nursing practice. Attained 10 CEUs Adheres to department/organizational policies and guidelines.
