The Case Manager is assgined to specific Hospital Units to facilitate the care plan for the inpatient episode, proactively monitor clinical quality and facilitate timely discharge. Using a collaborative interdisciplinary approach, the Case Manager will confirm the plan of care and focus on anticipation, integration and coordination of clinical care and discharge planning services necessary to meet the needs of patients in order to optimize clinical outcomes and resource utilization.
•4 year Bachelors/Masters degree preferred
WORK EXPERIENCE REQUIREMENTS
•1-3 years RN experience in acute care hospital
•1-3 years Case Manager experience in acute care setting and/or Managed Care Preferred
Penrose-St. Francis Health Services is a full-service, 522-bed acute care, two hospital system in Colorado Springs which includes Penrose Hospital and St. Francis Medical Center. With 364 licensed beds, Penrose Hospital provides secondary and tertiary (high tech) medical-surgical services with emphasis on elective and outpatient care. St. Francis Medical Center opened in August 2008. This 172-bed facility is the only full-service hospital in Northern Colorado Springs and features a modern Birth Center, Level IIIA Neonatal Intensive Care Unit, Pediatric Care Unit, Emergency Department, Level IV Trauma Center, Imaging Services, Surgical Services, and Critical Care Unit. It is also the home base for the Flight For Life Colorado air ambulance helicopter service.
Additional services include home care, urgent care, behavioral health, outpatient rehabilitation and community wellness programs.
