You have a gift for looking at signs and symptoms and honing in on what a patient needs—in physical and psychosocial terms, as well as from their family and community. You’ll discover many opportunities to apply that talent in the home care division at Holy Name Medical Center, a 361-bed Magnet hospital serving Bergen County, NJ.
In this position, you will perform initial and ongoing assessments, identify disease and nursing interventions, and develop an individualized plan of care. Additionally, you will serve as the focal point of care, communicating with clarity to patients, families and other health team members, and inspiring new independence and progress for each case. Responsibilities will also include, handling documentation, instructing, securing home medical equipment, and making referrals to community resources.
To qualify, you’ll need a current NJ RN license, outstanding assessment skills, and at least one year of experience in an acute care or home care setting. Candidates should also be self-directed and flexible to work on call and weekend rotations. BSN is desired.
