Care Coordinator, Outcomes Management (RN)
In this role you’ll coordinate and facilitate services to ensure the absolute best in disease management, outcomes, length of stay/utilization, and discharge planning, as well as efficiency across the continuum of care. You’ll interact with patients, families, medical staff, insurance companies and community agencies, providing counsel, education, information and careful analyses. You’ll also collaborate in the development of action plans, discharge plans, and overall care management.
The qualified nurse has a current RN license, along with superior analytical, communication and problem solving skills. Specific experience (3-5 years) with discharge planning, utilization review and Interqual and Milliman guidelines are required, while a BSN and CCM are desired.
