We have an exciting opportunity to join our team as a Nurse Practitioner in Wound Care/Ostomy Department. To qualify you must have a Masters degree in Nursing, current registration as a Professional Nurse in NY and registration as a Nurse Practitioner in NY, Family/Acute/Adult Nurse Practitioner Certification with prescriptive privileges, BLS with a minimum of five years of relevant, acute care experience. The successful candidate will be responsible for ordering customary laboratory, radiological and diagnostic studies per practice protocols; differentiating between normal/abnormal findings and following protocols for abnormal findings; formulating plan of care, along with the attending physician, patient and family, based in expected goals of care and length of stay; prescribing in-patient and discharge medications according to NYS law, including controlled substances with DEA authorization; performing complete health history, physical examination and psychosocial assessment of patients within the designated practice or clinical unit; reviewing abnormal lab, x-ray, and EKG results and determining if there is need for additional follow-up; prescribing a change in antibiotics when applicable, and contacting patients that fall into high risk groupings within 12 -18 hours of discharge to validate evolving improvement in their health status post discharge; plus all related job duties as assigned. Excellent communication skills are essential.
Membership in pertinent professional organizations preferred.
