As the Field Intake Nurse, you will: - Promote quality patient access to care, cost effectiveness and choice within a well-integrated healthcare delivery system
- Oversee the on-going assessment and evaluation of the patient condition and family situation, as well as the program personnel and vendors who are providing services to the patient
- Communicate these finding to the in house nurse case manager
