This is your opportunity to get in on the ground floor of an exciting initiative that is transforming the delivery of care. At Continuum, we use the title Care Manager instead of case coordinator or case manager because your role is caring for patients, not managing cases. Our Care Managers are responsible for ensuring that patient centered care is managed and delivered with the whole patient in mind. We are currently in the midst of implementing a new Medical Home Initiative (MHI) that will transform our affiliated primary care practices into fully functional Patient-Centered Medical Homes. This initiative, which incorporates best practices is aimed at improving performance on the key dimensions of quality, cost and patient satisfaction, and to prepare for the emerging healthcare environment. As the Medical Home Initiative continues to expand, we are seeking multiple Care Managers to organize, coordinate, and provide care coordination and care management services to patients who are most at risk for health deterioration, sentinel events, and/or poor outcomes. Care Managers will also ensure that patients receive optimal and preventive care across multiple health settings and multiple physicians/providers. Requirements: Requires a Registered Nurse (BSN required); and at least 3-5 years of acute care experience, preferably within the ICU setting; along with discharge planning, case management, and disease management experience. Position also requires excellent skills in: clinical assessment, EMR documentation, setting priorities and developing care plans. Communication, problem solving, and the use of population registries and other databases is essential. Experience with clinical system design, and project management skills are a plus; as is exceptional people skills to build relationships with patients, staff and providers.
